Our Services

Homemaker Personal Care

Assisting the individual with activities of daily living, dressing, feeding, transfer, ambulatory needs, or skills development.  May also include cooking, cleaning, laundry, money management, and shopping among others.

Adult Family Living

Personal care and support services provided to an adult by a caregiver who is related to and lives with the individual receiving the services.  Adult family living is provided in conjunction with residing in the home and is part of the rhythm of life that naturally occurs when people live together as a family.

Adult Foster Care

Personal care and support services provided to an adult caregiver who is not related to and lives with the individual receiving services.  Adult foster care is provided in conjunction with residing in the home and is part of the rhythm of life that naturally occurs when people live together as a family.

Social Work

Used to assist individuals and their families to improve and or restore their capacity for social functioning.  Social work includes provision of counseling and active participation in problem-solving with individuals and family members, counseling to meet the psychosocial needs of the individual.

Community Respite

Services provided to an individual unable to care for themself furnished on a short-term basis because of the absense or need for relief of those persons routinely providing the care.

Transportation

Transportation that is used by individuals enrolled in individual options, level one, and self-empowered life funding waivers solely to get to and/or from a place of employment or access to adult day support, integrated employment, supported employment, community supported employment and/or have vocational habitation.

Supported Living

Services provided for as long as 24 hours a day to an individual with a developmental disability through any public or private resources.  Including moneys from the individual that enhance the individual's reputation in the community  life and advance the individuals quality of life.

Support Brokerage

Responsible, on a continuing basis, for providing an individual with representation, advocacy, advice, and assistance related to the day-to-day coordination of services in accordance with the individual's plan.
